Consumer Health Data Privacy Policy

This policy applies to residents of Washington, Nevada, Connecticut, and other states with consumer health data laws. It describes "consumer health data" that Perfection in Parenting LLC dba Harmony at Home by Dr. Luisa ("Harmony at Home," "we," "us") collects through getharmonyathome.com and screener.getharmonyathome.com. It does not cover clients' clinical records, which are governed by our Notice of Privacy Practices. Our Privacy Policy describes our other information practices.

Consumer health data we collect

Your child's age and the concern you choose in the Screener (for example, attention, anxiety, mood, or development).
Your yes/no answers to the Screener questions and the resulting concern level.
Whether any answer indicated a possible safety concern.
Anything about your child's or family's health that you write to us through a form or email.

Why we collect it

To provide Screener results, follow up with support resources if your answers indicate a safety concern, support Dr. Luisa's review and possible future consultation, and send you information about our services related to your concern.

Where it comes from

Directly from you, when you use the Screener or contact us.

Whom we share it with

GoHighLevel, which hosts the Screener and stores your contact information, Screener answers, and results on our behalf.
Jane App, if you book a consultation.
Professional advisors, or authorities when required by law or to protect someone's safety.

We do not share consumer health data with analytics or advertising companies, and we do not sell it.

Your consent

You provide consent by starting the mental health screener and entering your personal information. We will ask again before collecting or sharing consumer health data for any purpose not described here.

Your rights

You can ask us to confirm whether we hold your consumer health data, to see it and the list of companies we've shared it with, to delete it, or to withdraw your consent. Email hello@getharmonyathome.com or call 303-900-8380. We'll respond within 45 days. If we deny your request, you can appeal by replying to our response, and we'll answer within 45 days. If you're not satisfied, you can contact your state attorney general.
